Prerequisites
To add a form to an existing page, you need a Form ID that defines where responses will be stored.
- Confluence Admins can create Form IDs for any space, including advanced destinations like email relay or Jira.
- Space Admins can create Form IDs for their own space, but these are limited to the internal database.
If you don't have the right permissions, contact your space or Confluence admin.
When to Use a Form in an Existing Page
Adding a form to an existing page is useful when:
- You want to capture feedback directly in documentation.
- You're gathering approvals or input on a project page.
- You're running a survey or poll as part of an ongoing discussion.
This approach keeps the context and the form together, making it easier for users to provide relevant responses.
Add the Forms Macro
At this point, the form is linked to your chosen Form ID, but you still need to add the fields you want to build your form.
Add Your Questions
Inside the Forms macro, insert the input field macros you need, for example:
- Input Field for short answers.
- Textarea for longer text.
- Radio or Checkbox Groups for multiple-choice questions.
- Dropdowns,Star Ratings, or Conditional Fields for richer surveys.
Each field macro you add will be tied to the Form ID you selected.
Save and Test
Once you've added your fields, save the page and submit a test response. This ensures the form is working as expected and responses are correctly linked to the results database or destination you configured.
