Use this macro to group related fields under a titled block.

Use this macro to group related fields under a titled block. You can also use it to draw a border around the section which is great for visually segmenting a single-page form. This is best used in short-to-medium forms that benefit from clear sections, but do not require questions to be split into separate pages.

Create your form using the Forms macro.
Add the Fieldset macro to the form in which you would like the fieldset to appear.
Give the Fieldset a Name.
Tip: The Name field is used to select fieldsets with the Conditional Field macro. This lets you show/hide groups of question fields based on earlier answers in the form.
Optional: In the Macro Editor, enter the Legend Text.
This is the title that will show at the top of the fieldset.
Select Show Fieldset Border to create a border around the group.
Click Save.
Add input field macros inside the Fieldset macro to build out your form.
Optional: Insert more Fieldset macros in the Form to create more groups.
Publish the page.
On saving the form, the legend text and the fieldset borders can be seen grouping the elements in the form.
