Destinations List

Explore Destinations List in Forms for Confluence Data Center.

The Destination List macro lets you add extra recipients for form submissions. there are in addition to the destination defined by the Form ID. When respondents complete the form, they can specify where their response will be sent, overriding the default form destination specified in the Form macro. This can help you streamline the number of forms you need.
  1. Create a form using a Forms macro.
  2. Enter a value in the Destination field. This acts as a default for any options in the Destination List that don't provide an alternate destination.
  3. Add a Destination List macro inside the Forms macro.
  4. Enter a List Label for your destination list.
  5. Optional: Enter a List Description for your destination list.
  6. Optional: Use the Destination Required checkbox to require a destination for each list item. If unselected list options can be created without a destination, these options will only use the destination linked to the Form ID. By default this check box is selected.
  7. Optional: Create your destination list options by completing the following fields:
    • Label - this what appears in the dropdown list for user to select.
    • Destination - If the Destination Required check box is selected this field is required and identifies the additional destination the form response is sent to when the list item is selected. If left empty the form response is only sent to the destination identified by the Form ID.
    • Subject - the subject line for the email sent containing the form response.
    • Success message - a custom success message shown when the form is submitted.
    Note: If the Destination, Subject, or Success Message are left blank, the corresponding value from the Form macro is used. If no success message is set (via a Form ID or a success macro the on-page), the default success message is shown.
  8. Click Save.
  9. Publish the page.
