Forms for Confluence makes it easy to build custom forms, surveys, and polls directly inside Confluence pages. Whether you’re collecting feedback, handling requests, or running an internal survey, Forms for Confluence helps you capture information where your teams already work.
Because forms live inside Confluence, responses can be saved in Confluence for reporting and analysis or sent to the right people by email or Jira integration. This means you can use forms for quick feedback, structured requests, or automated workflows, all without switching tools.
How Forms for Confluence Works
A form is created by adding the Forms macro to a Confluence page. Inside that macro, you place other input field macros to define your questions. With over twenty available macros you can create simple contact forms or complex multi-page surveys.
Once the page is published, users fill in the form as part of their normal Confluence experience. Submissions are collected instantly and can be:
Stored in Confluence for later export and reporting
Sent by email for fast notifications
Routed into Jira (with ScriptRunner) for automated follow-up
