Welcome to Forms for Confluence. This guide will help you create your first form, survey, or poll on a Confluence page.
If you're new to the app, it helps to remember that every form has two parts:
- The front end, where you design your questions.
- The back end, where results are stored and managed.
In this section, we'll focus on building the front end of a form and linking it to a results store automatically.
Create a Form from the Create Page Menu
To create your first form:
Start with a Template or an Empty Form
When creating a form, you can either:
- Use a template with pre-configured questions (e.g., Contact Request, Event Survey).
- Start with an empty form to build everything yourself.
If you select the empty form, you'll see a single Forms macro placeholder. This acts as the container for all the field macros you'll add.
Templates include several macros by default, which you can customize, remove, or replace.
Add Questions with Macros
Every question in a form is created using an input field macro. For example:
- Use Textarea for open-ended responses.
- Use Radio Button Group or Dropdown List for multiple-choice questions.
- Use Star Rating for quick feedback scores.
- Use Conditional Field if you want questions to appear based on previous answers.
Save and Test Your Form
Once you've added your macros, click Save on the page. Your form is now ready to be tested.
It's a good practice to submit a few test responses before sharing the form more widely. Testing ensures that your fields are capturing the right data and that your results appear in the correct place.
Keep Track of Your Forms
When you create your first form, a new page called "Forms for Confluence" will also be added to your space. This page acts as a central hub, letting you:
- See all existing forms in the space.
- Create new forms (each will be added as a child page).
This helps you keep everything organized in one place.
