The Confluence Export to PDF functionality does not work with apps that generate dynamic content. When a page containing certain macros is exported to PDF the content of those macros are not in the PDF. We have logged this issue with Atlassian https://jira.atlassian.com/browse/CONF-38112.
This currently affects these Adaptavist apps:
Forms for Confluence
Forms for Confluence Cloud
Community Forums for Confluence
Content Formatting for Confluence
To export your Confluence page to PDF we suggest you use your browser’s built-in functionality until a fix is in place for this. This is how you can use your browser’s print function to save webpages as PDFs.
Go to File>Print
In the Printer dropdown, choose Microsoft Print to PDF
Give the file a name and save in your preferred location.
Earlier Windows systems do not have a built in print to PDF function but you can use Adobe Acrobat or a PDF converter program by following these instructions from Digital Trends.
Alternatives to the DOPDF program recommended by Digital Trends are discussed by How To Geek in their explanation of printing to PDF in Windows.
Macs have built-in functionality to print to PDF
Open the document, web page, or file you want to print to a PDF.
Click the File menu and select Print, or just hit Command+P
.
Look for the PDF button in the lower left corner, click on that drop-down menu, and select Save as PDF.
Click Save in the save dialogue box, and save the file to whatever location you want (Documents is the default.)

Chrome has its own print to PDF function for both Windows and Mac. See the Adjustable Print Settings in these instructions from Google Chrome Support.
Click on File>Export to PDF.
You will get a Save As screen, where you can change the file name and where you wish to save it.
