A Form ID is how entry data is organized and stored in Forms for Confluence Cloud. A Form ID is created as a unique identifier to be used when creating a form and gathering data on a page.

A Forms for Confluence Cloud administrator should set up a Form ID as the response database before a page using a form is published.

A Form ID is required to create forms. A specific Form ID will be requested when adding Forms for Confluence to a Confluence page.

Create a Form ID

  1. Click Forms for Confluence to access the Forms Administration screen from a space where you want to store responses.

  2. Click Create Form ID.

  3. The Create Form ID window appears.
Provide a unique name in the Form ID field.

    1. (Optional) Add a brief statement about the form in the Form Description field.

    2. (Optional) Change the response restrictions in the Form Data Access Restriction field.
  4. Click Save ID. The window closes.

  5. A success message appears, and the new Form ID appears in your list.
You can now use this Form ID when adding a Form to a page within the same space.

Close the dialog window to return to the Administration page, or click Create Form to open a page and use the new Form ID.
