Checkboxes

Checkboxes should be used when you want your users to select one or more options from a set. Checkbox button groups let you quickly set a group of checkboxes for your users to choose from.


Before adding Checkboxes, or any other question, the Forms for Confluence container must be added to the page.

How to add a Checkbox to an existing form:

  1. Navigate to the page with the relevant form and enter Edit mode.

  2. Click the existing Form, then click Edit.

    The form configuration appears.

  3. Use the Add question button to add the Checkbox option.


    The checkbox configuration options appear.

  4. Fill in desired configuration details.

    1. Provide a title or question.

    2. Provide a Label for each option.

    3. (Optional) Add a description to appear under the checkbox options.

    4. (Optional) Click the icon to add a field. Use the option to remove one.

    5. (Optional) Use the Selected option to predefine a selected checkbox option.

    6. (Optional) Select the Make this field required checkbox option. 
      This will prevent users from submitting the form without adding an attachment in this field.

  5. Click Save in the Form builder when finished.
    This saves the inputs and returns the screen to Confluence Edit mode.
  6. Publish or Preview the page to see the final result.

Example use case:

Add the Checkboxes option to a form to gather data for an upcoming company lunch.

  1. Navigate to the page with the relevant form and enter Edit mode.

  2. Click the existing Form, then click Edit.

    The form configuration appears.

  3. Use the Add question button to add the Checkbox option.

    The checkbox configuration options appear.

  4. Click Add a question.

    Type: What is your favorite fruit?

  5. Click Add a description.

    Type: Please select one or more of the provided options.

  6. Click the first checkbox option, labeled Option.

    Type: Apple.

  7. Click the option.

    A new checkbox option appears below the first.

  8. Click inside the Text field of the next option.
    Type: Lemon

  9. Repeat clicking the option to add a checkbox option twice more. Label the checkbox options created as Kiwi and Pineapple.

  10. Ensure the Selected option remains on the first field, labeled Apple.

  11. Click Save.
    The field appears as a placeholder in Confluence Edit mode.

  12. Click Publish or Preview to see the result.
